SUMMER KARATE DAY CAMP
Join us as we spend the summer together with Mr. M and Instructors at Hapkido USA's Karate Day Camp. There will be lots of activities during the 9 week program.
Your child can drop in for 1 day or the entire 9 weeks. Hapkido USA's Karate Day Camp offers daily sessions with specialized instruction in kids' Martial Arts, trained by our Master Instructor Jino Kang and Chief Instructor Mr. M. All of our instructors are personally trained and certified Black Belts by Master Kang. Each instructor has more than 10 years experience at Hapkido USA and we do not hire people outside of our school. Our instructors and camp leaders are always courteous and professional.
Our Martials Arts program is designed to teach your child important life skills and physical fitness in a fun and challenging way. Our proven method of character building uses Martial Arts to build confidence, discipline, focus and increase physcial fitness for more positive and healthy lives.

RELEASE OF LIABILITY AND INDEMNITY AGREEMENT
(Please Read Carefully Before Signing)
As a prior condition for the use of the facilities, to receive instruction from, and to participate in any activities of HAPKIDO USA, LLC (“HUSA”), including but not limited to Summer Camp on or off premises, at this time and on all future dates, the undersigned (referred to hereinafter as “Student”) and all heirs, assigns, and legal representatives agree to the following:
ADVISORY OF STUDENT’S RIGHTS AND RESPONSIBILITIES
Safety is not the sole responsibility of the instructors and staff. Every student is responsible for their own safety and the safety of those around them. All students have the right and responsibility to excuse himself/herself from any exercise or activity they believe
will be harmful to them. All students may evaluate each situation in the context of their own skill and current physical condition, and conduct each exercise or activity in a manner that is safe. If an instructor asks a student to perform an act which the student believes
is beyond his/her capability to do safely, or which the
student believes is riskier than he/she is willing to hazard, then it is Student’s responsibility to inform the instructor and to refrain from such an activity. (Parents/Guardians, please discuss
these
rights and responsibilities with your child.)
ASSUMPTION OF RESPONSIBILITIES AND RISK
The Martial Arts are a potentially dangerous activity. Bumps, bruises, scrapes, scratches, minor muscle strains, and soreness are commonplace. Most students will encounter these sorts of minor injuries from time to time in their training
More serious injuries
are possible, including, but not limited to, strains, sprains, twists, cramps, and injuries of a similar magnitude, and students can expect to encounter these injuries infrequently. The possibility of even more serious injury
exists, including, but not limited
to, torn ligaments, fractured bones, and broken bones, though few students encounter such serious injuries. Furthermore, in the Martial Arts, despite any and all safety precautions, there remains the
possibility of crippling or death.
RELEASE
Student understands the above statement of risk, and understands the rights and responsibilities of participating in HUSA activities. Student assumes responsibility for his/her own safety (or on behalf of a child, the child’s safety), understanding and accepting
the risks involved with Martial Arts training and related activities. Even if the instructor (s) informs of no serious injuries having ever happened in HUSA classes or events, Student understands that this does not mean that there is no possibility of harm. By assuming this risk,
Student completely absolves HUSA, all owner (s), instructors, staff, guests, students, landlords, management companies, and any and all other parties of liability for any harm, unless that harm is intentionally caused by wrongful conduct.
In the event of harm
intentionally caused by wrongful conduct, liability will extend only to the individual (s) who’s intentional and wrongful conduct caused the harm.
WAIVER
Student waives the protection afforded by any statute or law in any jurisdiction including California Civil Code 1542 which purpose, substance and/or effect is to provide that a general release shall not extend to claims, material or otherwise, which the person
giving the release does not know of or suspect at the time of executing the release. This means, in part, that the undersigned is releasing unknown future claims.
INDEMNIFICATION BY STUDENT (OR BY PARENTS/GUARDIAN)
Student agrees not to bring any claim or suit against HUSA, HUSA’s owner (s), instructors, students, staff, guests, landlords, management companies and any and all other parties on behalf Student (or child), for any injury or harm sustained by any event short of
a grossly negligent, intentionally wrongful, or criminal act, and in these situations, only the individual (s) having the specifically wrongful intent or gross negligence shall be the subject of such a claim. Student further agrees that he/she will not cause to
be brought, nor encourage a claim or suit based on activities which are within the normal practice of Martial Arts, which may have a chilling effect on the normal and reasonable practices of instruction in a potentially hazardous sport. Student also agrees not
to cooperate in the bringing of such a suit or claim except insofar as legally required to do.
Student shall indemnify HUSA, its owner (s), instructors, staff, students, guests, and any and all additional defendants covered by this Agreement for all judgments, costs, attorney fees, and other expenses incurred as a result of a breach of this Agreement or
any suit or claim for injuries or damages as a result of participation of any HUSA activity by Student.
SEVERABILITY
If any clause, sentence, phrase, or statement is found unenforceable or invalid by a court of law, the remainder of this document shall remain valid and enforceable.
DURABILITY
This document is effective from the date signed with no expiration. Furthermore, the terms of this document are retroactive to the beginning of participation in HUSA activities, if this document was signed after such date.
ADVISORY OF HAPKIDO USA’S RIGHTS
HUSA may terminate the membership of any student at any time. Student agrees to abide by the rules and regulations of HUSA.
Student has read and understands this Agreement in its entirety.
